Under the WA School Education Act 1999, students who have regular appointments during school term time are required to seek the written permission of the school principal. If your child will be absent from school on a regular basis due to medical appointments, we request that you use the following form which must be submitted for approval prior to attending these appointments and which then will need to be approved by the principal.
Please note that the form is only valid for the year that it has been applied for and approved and a new application must be made each year.
